We take your privacy very seriously, and the information that you provide us on these forms treated accordingly.  Your private information will be transmitted to us using modern high level encryption technology to protect your privacy, and cannot be read or intercepted by others.  The information will then be placed in your chart in our office, and this is the only place that it will be stored.  It will not be stored in a database, nor will it be stored anywhere else electronically.
